Guilderland – Phyllis M.Tvorak, 75, passed away peacefully with her family by her side on March 3, 2017 at Our Lady of Mercy with Hospice care. Phyllis was the daughter of the late Wilbur and Mae (Bellows) Woods.

Phyllis was born on June 5, 1941 in Tupper Lake, NY. Phyllis was a retired LPN, nurse. In her free time she loved to work on craft projects, garden outside and was always ready with a "Do you want to play?", her favorite game of Tri-Ominos. She was a dedicated loving mother who enjoyed being with all of her grandchildren; whether it was babysitting them or watching as they played their various sports.

She is survived by her son Richard Tvorak and his partner Judi, Cheryl (Vincent) Futia and Mae (Robert) Mohr; her beloved grandchildren: Heather and Richard Tvorak ; Cody, Jesse, Katherine and Dalton Futia; Elizabeth Paiz; and Danielle and Mackenzie Mohr; and her beloved great grandchildren: Liam Vincent and Amelia Paiz. She will be missed dearly by her lifelong friend Barbara Shunk and her BFF Millie Futia.

In addition to her parents, Phyllis was predeceased by her loving daughter Terri Hoyt in 2011.

Phyllis requested that there be no wake or services, she wanted to be cremated and brought home to Tupper Lake. The family will honor her request with a private service in Tupper Lake this spring.
